We provide support for activities of daily living such as bathing, grooming, dressing, and toileting to help your family member maintain their independence and dignity in their own home.

• Determination of home safety concerns
• Referral to Independent Home Health Aide (assisted matching for your specific need)
• Subsidize cost of independent home health aides, As Available
• Help ascertain home medical supplies and equipment (as available through donation program)
• Provide an Individual plan (road map) to help independent home health aide achieve our client’s goal
• Continued communication with families or responsible parties
• Referral to other needed community resources and/or agencies
